Please feel free to share the form with friends & family. You are also more than welcome to pick up form(s) from our church office. 

— Registration forms will not be sent home in paws folders as our registration deadline will end before school begins. 

— Registration forms will be made available for pick up at the John L. Street Library. 

— You may turn your form(s) back into  the church office and/or our soccer mailbox at the church. That is the black mailbox that is a locked drop box used specifically for registrations and located at the front entrance of the church. 

—Forms and fees may also be mailed back in to the following mailing address: 482 Lakota Drive.
